Adoption/Foster Form

~~~ ADOPTION/FOSTER APPLICATION~~~
 As proud members of The Mayor’s Alliance for NY City’s animals, our goal is to place our homeless rescues in permanent loving homes, as their former owners have already abandoned them at least once before. As we strive to place the appropriate dog in the appropriate home, this application helps us to learn more about you, your home and your level of experience, if any, with dogs.
ALL OUR RESCUES ARE SPAYED/NEUTERED PRIOR TO ADOPTION.
Please fill out ALL questions. If any question does not apply, write “N/A”.
Our adoption fee is $300 (puppies) $225 (dogs over 6 months) $175 (dogs over 3 years). Any donation or foster home is always welcome, and very much needed. All donations and adoption fees are applied toward caring for the dogs, vet bills, medications, follow-up visits, rechecks and boarding expenses we incur.
  24. The foster/adoption proceduring including filling out this application, a phone interview, meet the dog/dogs, in-person interview, contract signing (which stipulates the animal MUST be returned to us if you can no longer care for him/her), and home visit prior to fostering/adopting. This is all to ensure the safety of the dog since we don't know you. Is this acceptable?
